Ms. Sarah Ellis was admitted at Restore Health Rehabilitation Center following a short hospital stay due to a fall at home. Unfortunately, the fall affected Ms. Ellis’ overall functional independence, strength and mobility preventing her from returning home immediately. Ms. Ellis lives alone in Senior apartment where she enjoys joining different activities and program they have for the community. The Reliant Rehab therapy team and Sarah got to work on developing a plan of care specific to her goals and needs to get her back to enjoying life at home as quickly as possible.

Ms. Ellis worked hard in therapy and made excellent progress, achieving her goals of returning home, achieving independence in all her functional care tasks, mobility and even showed a significant improvement in her overall strength! She is very happy to be able to return to her home and we wish her the best of luck and congratulations on her success!
